• The risk of medication errors is a latent problem in day-to-day hospital routine. There are a number of risk sources which can ultimately have severe consequences for the patient. In addition to incorrect medication due to connection errors, e.g. in the case of neuraxial and intravenous applications, unstructured labelling of syringes can also result in very serious medication mix-ups.
• The up-dated DIVI Recommendation from 2012 addresses this problem and refers to the international standard ISO 26825. The concept of this standard is to classify drugs into drug classes, each of which is assigned a unique colour code. In this way, the most severe medication errors – resulting from mixing up drugs from fundamentally different pharmaceutical effects – are inhibited.
